Obviously the people on the boat are struggling to keep the boat steady in the torrent of water coming over Niagara Falls.
In the inside the inscription essentially says that "Great fortunes are sure hard to find but you've really done it."

This is a great sentiment of congratulations and acknowledges the great accomplishment of the person rather than the fortune coming from luck. This card probably is not appropriate for someone who wins a lottery prize.
